FILE A CLAIM
Pursuant to the Court's Order Preliminarily Approving Settlement and Providing for Notice filed on August 13, 2020, if you are a Class Member and wish to be potentially eligible for a payment from the proceeds of the Settlement, you must complete and submit a Proof of Claim and Release ("Proof of Claim"). To obtain a copy of the Proof of Claim, click here. The deadline to submit a timely Proof of Claim is January 4, 2021.
You may submit your Proof of Claim using either one of the following methods. Each method is described below, with links to the requirements for each submission. Your submitted Proof of Claim will include a certification under penalty of perjury that your Proof of Claim is accurate and complete.
Whichever submission method you choose, you are required to provide documentation to support your claimed transactions. Please remember to provide supporting documentation of all transactions listed on your Proof of Claim. In the event that you submit your Proof of Claim online, you must retain confirmation of such filing.
If you fail to execute and submit a timely, properly completed and addressed Proof of Claim, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.

SUBMIT YOUR PROOF OF CLAIM BY MAIL
To submit a Proof of Claim by mail, complete and sign the Proof of Claim in its entirety, with all required supporting documentation and mail it, postmarked no later than January 4, 2021, to the Claims Administrator at:
